I recently had a patient who underwent colonoscopy for anemia. He was a 60 year old male with constipation and the referring physician asked to have the procedure performed. The patient did not have previous screening colonoscopy done. During the procedure, we found a 4.0 cm pedunculated polyp with surface features still consistent with an adenoma and the stalk of the polyp devoid of dysplastic changes based on image enhanced endoscopy. Here I would like to share the video on how polypectomy of a pedunculated polyp is performed. In the video, I deployed an endoloop but after cutting the polyp, the loop fell off so I deployed 3 hemoclips to decrease the risk of bleeding. Polyps with this size usually have a large feeding vessel so it was decided to clip the base since the endoloop fell off.
The histopathologic analysis of the resected polyp turned out to be tubulovillous adenoma with no dysplasia in the resected base.
